Billing Requirements for Emergency Department Services

Meridian would like to remind providers of requirements for billing Emergency Department services.

HFS guidelines require General Acute Care Hospitals enrolled in IMPACT with category of service 24 to bill emergency department services with Revenue Codes 0450, 0451 or 0456 along with the associated CPT®/HCPCS codes outlined below.

450	99284, 99285, 99291, G0383, G0384
451	99282, 99283, G0381, G0382
456	99281, G0380

Claims billed with no supporting CPT®/HCPCS or codes not listed above will deny EX1H – Deny: ER Reve Code & CPT/HCPCS combination invalid*.

At least one of the 45X revenue codes must be billed with one of the associated CPT®/HCPCS listed. If revenue code 45X is billed on an IP claim without the appropriate CPT®/HCPCS, the entire claim will also deny.
