Meridian Encourages Member Participation in Diabetes Prevention and Self-Management Education and Support Programs

Meridian wants to assist providers in battling the rise in diabetes and prediabetes. With that in mind, we are pleased that services delivered by accredited Diabetes Prevention Program (DPP) providers and Diabetes Self-Management Education and Support (DSMES) providers are covered by Medicaid.
Diabetes Prevention Program (DPP)
As you may know, the DPP is part of a national program recognized by the Centers for Disease Control designed to raise awareness about the seriousness of prediabetes and provide resources for those diagnosed with it. With a multitude of resources, the DPP has made encouraging patient participation relatively easy. Visit chicagocaresdpp.org, to download a toolkit with everything you need to bring awareness to your patients. Prewritten social media posts as well as educational material such as a customizable newsletter, flyers, brochures, and videos are available to download.
Diabetes Self-Management Education and Support (DSMES) Program
Individuals with all types of diabetes, including Type 1, Type 2, or gestational diabetes, may benefit from a DSMES program, which aims to provide information and tools to help them manage their diabetes, avoid serious complications, and improve their quality of life.
