MERIDIAN OFFERS CARE COORDINATION PROGRAM THAT HELPS MEMBERS MEET THEIR HEALTH GOALS

Our Care Coordination program can help your member get the right services in the right setting at the right time! Care Coordinators use a holistic approach to connect members to services and resources needed to meet their individual healthcare goals. Some of the goals of Care Coordination are to:

• Conduct a Health Risk Assessment
• Develop and implement an individualized plan of care
• Provide self-management education
• Help navigate through health care episodes including chronic conditions, episodes of care, and pregnancy
• Increase medication adherence
• Ensure safe transition of care

If you have a member that would benefit from enrollment in our Care Coordination Program, you can submit a referral in three ways:

1. Notifying Meridian through the provider portal
2. Completing the Care Coordination/Complex Case Management Referral Form (PDF)
3. Requesting a Care Coordination Referral Form from your local Provider Network Development Representative.
