TERMINATION OF SOMATUS CARE MANAGEMENT PROGRAM

Effective July 1, 2023, Meridian will no longer partner with Somatus Inc., a provider of care management services for chronic kidney disease (CKD) and end-stage renal disease (ESRD). Some of your patients may be affected by this change.

Consequently, some Meridian members may be enrolled in our care management program, the Centene CKD Center of Excellence. This program will continue supporting care management needs for members with CKD and ESRD.
