Prior Authorization Code Updates

As you know, MeridianHealth’s prior authorization (PA) submission process and codes have changed.

After evaluating system performance, Meridian has decided to make some additional changes to the PA list. Please note: Those changes will be effective within 60 days of this notice, by October 11, 2021.

Please refer to our prior-authorization check tool for any code requirements.

Prior authorizations should be submitted through the provider portal at provider.ilmeridian.com, or by phone or fax.
