Living with a chronic condition can be challenging. That’s why we started our Chronic Disease Management Program. This program offers support from nurses and lots of resources to help our members live brighter.
This program is for members with asthma, chronic obstructive pulmonary disease (COPD), diabetes, or heart disease. It is also for members who want to quit smoking.

How it works
Meridian's respiratory disease management program covers asthma and chronic obstructive pulmonary disease (COPD) conditions. The goal of the program is to help you manage your asthma or COPD symptoms and keep your disease under control.

Meridian's Diabetes Disease Management program is designed to support the treatment plan your doctor has developed for you and to assist you with controlling your disease.

Meridian's Heart Disease Management program can help you manage heart conditions and prevent long term complications. Our Heart Disease Management program can assist members with heart failure or coronary vascular disease (CVD).

Meridian is working with the State of Illinois to stop new HIV cases. The Illinois Department of Public Health is sharing HIV data they have with Illinois Medicaid and Illinois Medicaid Managed Care Organizations to have better care for people living with HIV. Name, date of birth, SSN, HIV status, and other information is being shared safely and securely for all Medicaid members.
