Throughout his childhood, Lucas overcame several health issues with the help of his mom, Nanci. But he struggled to manage his complex conditions as an adult. Worried, Nanci reached out to his care manager, Catherine. who helped get his health back on track.

Michael needed help when dealing with several barriers impacting his health. Damaria, his care manager answered his call. She connected Michael to the resources and support he needed most.

Noah was diagnosed with level 3 autism spectrum disorder early in life. His mother, Stephanie, needed help to address his food intolerances. And to keep him safe from harm’s way. That’s when his care manager, Joe, stepped in to help.

Steven’s health depended on the maintenance of an implanted port for blood drawing. His mother Debbie, who had cared for him since birth, needed help with obtaining maintenance of this device. It was thanks to the support of his care manager, Michael, that Steven received the care he needed.
